Payment Process

-Payment Due at Scheduling: Full payment is required when scheduling your appointment, with confirmation following successful payment processing.


- Payment Methods: We accept all major credit cards and debit cards for convenience. FSA and HSA cards can also be used for certain services. Please note that we are a cashless patient service center; we do not keep cash on hand and cannot accept cash payments.

Scheduling Process

- Appointment Scheduling: While it's preferable to schedule appointments online for efficiency, we understand you may have questions or specific needs. For general service inquiries and scheduling by phone, please call (678) 408-7363 ext 0 during normal business hours. For after-hour appointments or onsite visits for four or more employees, please dial ext 4.


- Time Zone: All appointments are scheduled according to Eastern Standard Time (EST).


- Service Reminders: Expect a reminder 24 hours and on the day of your scheduled service for your convenience.

Cancellation Policy


- Same-Day Cancellation Fee: Same-Day cancellations and No Shows will incur charges equivalent to the full cost of service. 


- 24-Hour Notice Requirement: We request rescheduling or canceling at least 24 hours before the appointment time to avoid a $50 fee and allow us to manage our resources effectively.


- Charges for Late Cancellations: Appointments not canceled or rescheduled at least 24 hours prior will incur a $50 fee which will be charge to the card on file from current booking. If a card have not been placed on file please note this fee will be applied to any future services


- No-Show Policy: Failing to attend a scheduled appointment without notice will result in a full charge of the scheduled service.

How to Cancel or Reschedule Your Appointment

To cancel or reschedule, please contact us through one of the following methods as soon as possible:

- Phone: Call us directly at the numbers provided on our website and in your appointment confirmation for immediate assistance.


- Email: Send an email to our designated appointment management address, detailed in your confirmation and on our website.


- Online: Use any online appointment management tools we offer via our website or a client portal.

Refunds and Credits

- Refunds: Typically, refunds are not provided. However, exceptions may be considered under specific circumstances at our discretion.


- Service Credits: In case of rescheduling, any payments made may be credited towards future appointments.

Use of Services

- Compliance: Clients must comply with our collection process and provide accurate information.


- Identification: Valid identification is required for all testing services to ensure service integrity.

Privacy and Data Protection

- Confidentiality: Protecting your privacy is our priority. All client information and test results are handled confidentially, aligning with our Privacy Policy.

Amendments

- Policy Changes: We reserve the right to amend these terms and conditions as needed. Changes will be effective immediately upon posting, underscoring the importance of regular review.

Acceptance of Terms
